Patient Information and Consent

Due to the current circumstances of the Coronavirus pandemic I have implemented a number of extra risk assessments, triage and hygiene protocols for all patients. It is important that all patients are aware of these procedures and understand the risks. These will explained to every patient. Every patient is required to read and sign a Covid-19 patient information and consent form. A copy is found HERE.

This form is in addition to the consent form for chiropractic examination and treatment. Please bring the printed and signed patient information and consent form with you on your first visit. If you don't have a printer copies of this form will be provided at the first appointment. The form provided must be read and signed before treatment can commence.

What to expect when you come to the clinic

  • Please go to the toilet before you start your journey.

  • When you arrive at Nettlebed Surgery stay in your car and text or ring me to say you have arrived. I will come out to your car, take your temperature and ask you a number of questions as part of the point of care risk assessment and triage.

  • When you arrive at Runwize Sports Wallingford please wait in the shop and I will come to you, take your temperature and ask you a number of questions as part of care risk assessment and triage.

  • Please wear a mask at all times. If you do not have a mask I will provide you with one for a small charge.

  • When I have risk assessed you and am satisfied you are low risk I will escort you into the building or room.

  • Please try to avoid touching anything in the building for hygiene reasons.

  • Bring into the building as few possessions as possible.

  • Wear lose fitting clothing. Nothing thick or heavy or more than one layer. This is so you don't have to get changed in the clinic.

  • Where possible you will be treated with the windows open for hygiene reasons.

  • You may bring with you a large clean bath towel to help keep you warm.

  • I will ask you to wash your hands or use hand sanitiser before signing the consent form.

  • If you require a chaperone they will have to go through the same Covid procedures and consent form. Please telephone me the day before the appointment if you wish to bring a chaperone with you.

  • Please bring the correct amount of cash or cheque with you

  • Please minimise your time spent in the clinic.
